CASE: PIVD L5–S1 | Minimally Invasive Spine Surgery 🦴
55-year-old female patient presented with low back pain radiating to the left lower limb.
Neurology, bowel & bladder functions were intact.
📌 Diagnosis:
Prolapsed Intervertebral Disc (PIVD) at L5–S1, compressing
➡️ traversing S1 nerve root
➡️ exiting L5 nerve root
📌 Procedure Highlights:
✔ Disc level identified under fluoroscopy (AP & lateral)
✔ Guide wire placed safely (without crossing posterior vertebral line)
✔ Working channel created over guide wire
✔ Endoscopic discectomy performed
✔ Compressed disc fragment removed with minimal tissue damage

Sterilization is a critical step in endoscopic and spine surgery instrument care.
Different instruments require different sterilization modalities depending on their material, sensitivity, and urgency of use.
In this video, we explain the four commonly used sterilization methods in the operating theatre and CSSD, along with their advantages, disadvantages, and practical use cases.
🔹 Sterilization Modalities Explained:
✔ Autoclave
• Uses high temperature & pressure steam
• Most commonly available and cheapest method
• Suitable for heat-resistant instruments
⚠️ May damage sensitive instruments & endoscopes
✔ ETO (Ethylene Oxide) Sterilization
• Low-temperature gas-based sterilization
• Suitable for heat-sensitive instruments
• Machine & process are cheaper than plasma
⚠️ Long cycle time (8–10 hours)
✔ Plasma Sterilization
• Low-temperature, gas-based method
• Short cycle time (30–60 minutes)
• Instruments available quickly for next surgery
⚠️ Most expensive modality
✔ Chemical Sterilization (Cydex)
• Instruments immersed for 10–15 minutes
• Useful for emergency or rapid turnover cases
⚠️ Sterilization reliability may vary
⚠️ Solution must be prepared fresh and used within 24 hours
